I'd speculate that perhaps Elvis doesn't survive his tumble down the stairs. That would incite Greene's rage. Perhaps he has the Bolivian police stop Bond's vehicle where Mathis is killed and Bond and possibly Fields captured. This leads into the plane sequence. I also think that Fields is indeed a traitor working for Greene but perhaps she failed him someway. That's the only way I can see getting away with giving her such a horrific end.

If it's his suite in Bolivia, why is he wearing the same suit as he is in the first picture, which we know to be Siena? I'd say the second shot is post-Siena, when Bond goes back to the safehouse. That's probably taken in an above-ground section ofthe safehouse, deliberately designed to look sterile so that if ever anyone strays into it, they'll think they're in a branch of some big company - like Universal Exports - rather than an MI6 safehouse. Everything else, like where Bond questions White and where the fight scene with Henry Mitchell takes place, would be underground.

At least, that's my guess.
